Job Description
PRIMARY PURPOSE/FUNCTION OF POSITION: Provide case management for clients in Genesis Program, coordinate Genesis Program mentors; and backup other ministry staff by providing case management for crisis care.
REPORTS TO: Executive Director
SUMMARY OF DUTIES:
1. Possess and effectively utilize an in depth knowledge of: social service/counseling principles, client management, and the various Sioux Empire organizations that provide resources to the people who contact us.
2. Review/approve Genesis Program applicants. (Verify applications, interview referral sources, perform in-home visits, etc.)
3. Work with Volunteer/Development Coordinator to ensure clients are well matched with mentors.
4. Provide case management and aftercare to Genesis clients.
5. Provide training, assistance, and oversight to mentors while active in the Genesis Program:
6. Backup and support staff providing direct services to clients, including: assessment, self-sufficiency planning, money management, advocacy, referrals, financial assistance, and follow up case management.
7. Analyze program effectiveness and recommend needed changes.
8. Other duties as assigned by Director.
Related critical skills and duties include:
• Factually obtain and concisely record client and mentor information in computerized database.
• Document client activity according to established department guidelines.
• Effectively refer clients to appropriate agencies for other resources.
• Cooperate with other agencies and professionals to coordinate services for mutual clientele.
• Follow all agency policies and procedures regarding distribution and recording of funds.
• Maintain confidentiality of client and agency information.
• Serve as a liaison to visitors and callers by relaying and providing accurate information in the absence of Receptionist.
• Provide accurate reports.
• Prepare for and actively participate in staff meetings.
• Periodically assist with special events and development activities.
• Periodically represent the ministry at community meetings and events.
Requirements:
HOURS:
Full-time position; requiring flexible scheduling to perform home visits, mentor training, and occasional public speaking opportunities.
EXPERIENCE/EDUCATION:
Requires Bachelors degree in counseling, social work, or related human service field and a minimum of one-year related work experience or an equivalent combination. All candidates must be at least 21 years of age. Effective communication and case management skills are necessary. Individual must possess a genuine desire to teach and guide participants and be a positive role model to all participants. Daily access/use of personal automobile, valid SD driver’s license, and appropriate vehicle insurance required.

applicable expenses reimbursed
